Its aim is to improve people\u2019s health literacy about COVID-19 by providing <a href=\"https:\/\/www.cdhn.org\/covid-19-fact-checks\" target=\"_blank\" rel=\"noopener noreferrer\">accurate and up-to-date information<\/a> which will increase knowledge, understanding and confidence and enable people to make good health decisions.&nbsp;FactCheckNI have also produced a live Q&amp;A article on shielding letters. It will be updated as and when new information becomes available &#8211; you can find it <a href=\"https:\/\/factcheckni.org\/fact-checks\/health\/shielding-letters-qa\/\">here<\/a>.]<\/em><\/p>\n\n\n\n<p><strong>Summary<\/strong><\/p>\n\n\n\n<ul><li>A \u2018shielding letter\u2019 is to inform those that they should comply with stricter stay at home restrictions in order to protect themselves from COVID-19.<\/li><li>GP Practices were provided with a search tool by the Health and Social Care Board (HSCB) to aid them in identifying those patients who were the \u201cclinically extremely vulnerable\u201d. GPs were also asked to use their clinical judgement as to who should receive a shielding letter based on the criteria listed. The guidance issued is advisory for the patient\u2019s health, but the UK government advises that if you choose not to follow these regulations, to speak with your GP before reaching the decision.<\/li><li>The Department of Health is working closely with Community Pharmacists and Community Health Development Network to ensure that those who are in need of medicines and are \u2018shielding\u2019 can avail of pharmacy deliveries.<\/li><li>Guidance on shielding for people at potentially higher risk from COVID-19 is being actively reviewed.<\/li><\/ul>\n\n\n\n<p><strong>What is shielding and a shielding letter?<\/strong><\/p>\n\n\n\n<p>While <a href=\"https:\/\/factcheckni.org\/fact-checks\/health\/what-are-the-rules-of-lockdown-in-northern-ireland\/\" target=\"_blank\" rel=\"noopener noreferrer\">\u2018lockdown\u2019<\/a> and social distancing measures have impacted all UK citizens, the <a href=\"https:\/\/www.gov.uk\/government\/publications\/guidance-on-shielding-and-protecting-extremely-vulnerable-persons-from-covid-19\/guidance-on-shielding-and-protecting-extremely-vulnerable-persons-from-covid-19\" target=\"_blank\" rel=\"noopener noreferrer\">\u2018clinically extremely vulnerable\u2019<\/a> are being \u2018shielded\u2019. They should have received a letter to confirm their status from their GP or from their Hospital Consultant. This \u2018shielding letter\u2019 is to provide further guidance on stricter stay at home restrictions in order to protect themselves from COVID-19.<\/p>\n\n\n\n<p>For those in this group, the government has advised them:<\/p>\n\n\n\n<ul><li><span style=\"font-size: 16px;\">Do not leave their house.<\/span><\/li><li>Do not attend any gatherings. This includes gatherings of friends and families in private spaces, for example, family homes, weddings and religious services.<\/li><li>Strictly avoid contact with anyone displaying symptoms of coronavirus (COVID-19). These symptoms include high temperature and\/or new and continuous cough or loss or change to your sense of smell or taste.<\/li><\/ul>\n\n\n\n<p>These additional social distancing measures are what is referred to as <a href=\"https:\/\/www.gov.uk\/government\/publications\/guidance-on-shielding-and-protecting-extremely-vulnerable-persons-from-covid-19\/guidance-on-shielding-and-protecting-extremely-vulnerable-persons-from-covid-19\" target=\"_blank\" rel=\"noopener noreferrer\">\u2018shielding\u2019<\/a>, and the government has advised those who are clinically extremely vulnerable to shield until the end of June. A template letter developed by the Health and Social Care Board for GPs to send to patients is available <a href=\"https:\/\/www.health-ni.gov.uk\/sites\/default\/files\/publications\/health\/gp-template-letter.pdf\" target=\"_blank\" rel=\"noopener noreferrer\">here<\/a>.<\/p>\n\n\n\n<p><strong>Who gets a shielding letter?<\/strong><\/p>\n\n\n\n<p>An original estimate of <a href=\"https:\/\/www.belfasttelegraph.co.uk\/news\/health\/coronavirus\/coronavirus-chief-medical-officer-warns-against-trivialising-outbreak-as-40000-to-be-told-to-shield-themselves-39067205.html\" target=\"_blank\" rel=\"noopener noreferrer\">40,000 people likely to be \u2018clinically extremely vulnerable\u2019 in Northern Ireland<\/a> almost doubled and <a href=\"https:\/\/www.youtube.com\/watch?v=ZHxBJzkYOqY\" target=\"_blank\" rel=\"noopener noreferrer\">80,000 patients should have received a shielding letter from their GP<\/a>. Groups include:<\/p>\n\n\n\n<ol><li><span style=\"font-size: 16px;\">Solid organ transplant recipients<\/span><\/li><li>People with specific cancers:\n<ul>\n<li>people with cancer who are undergoing active chemotherapy<\/li>\n<li>people with lung cancer who are undergoing radical radiotherapy<\/li>\n<li>people with cancers of the blood or bone marrow such as leukaemia, lymphoma or myeloma who are at any stage of treatment<\/li>\n<li>people having immunotherapy or other continuing antibody treatments for cancer<\/li>\n<li>people having other targeted cancer treatments which can affect the immune system, such as protein kinase inhibitors or PARP inhibitors<\/li>\n<li>people who have had bone marrow or stem cell transplants in the last 6 months, or who are still taking immunosuppression drugs<\/li>\n<\/ul>\n<\/li><li>People with severe respiratory conditions including all cystic fibrosis, severe asthma and severe chronic obstructive pulmonary (COPD)<\/li><li>People with rare diseases and inborn errors of metabolism that significantly increase the risk of infections (such as Severe combined immunodeficiency (SCID), homozygous sickle cell)<\/li><li>People on immunosuppression therapies sufficient to significantly increase risk of infection<\/li><li>Women who are pregnant with significant heart disease, congenital or acquired<\/li><li>People who have had a splenectomy<\/li><li>People undergoing renal dialysis<\/li><li>People with motor neurone disease<\/li><\/ol>\n\n\n\n<p><strong>Who sends the shielding letter?<\/strong><\/p>\n\n\n\n<p><a href=\"https:\/\/www.bbc.co.uk\/news\/uk-northern-ireland-52245055\" target=\"_blank\" rel=\"noopener noreferrer\">GPs were asked by the Health and Social Care Board (HSCB)<\/a> to use a search tool and their clinical judgement to decide who should receive a shielding letter based on the criteria listed. Health and Social Care Trusts issued shielding letters to some patients receiving consultant care.<\/p>\n\n\n\n<p>The HSCB has a central database of all of those individuals who have been asked to shield by their GP. The HSCB has also worked with the Department of Communities to provide a validation service for those who are shielding to enable priority access to booking online shopping.<\/p>\n\n\n\n<p>During a <a href=\"http:\/\/data.niassembly.gov.uk\/HansardXml\/committee-21959.pdf\" target=\"_blank\" rel=\"noopener noreferrer\">Committee for Health meeting on 2 April<\/a>, Health Minister Robin Swann stated:<\/p>\n\n\n\n<p>\u201cThe GP letters that we issued were from the first trawl of the database and the health records that we were able to get for those who were identified \u2026 What we are concerned about, and what the Health and Social Care Trusts are now doing, is making sure that the GPs have trawled all their systems and making sure that the Health and Social Care Board is going through its hospital records to make sure that nobody in those groupings has been overlooked or missed.\u201d<\/p>\n\n\n\n<p>If you are receiving chemotherapy, radiotherapy, specialist drugs only supplied by the hospital and\/or stem cell transplants, or under the care of a hospital consultant, the HSC confirmed to FactCheckNI that you are more likely to receive a shielding letter from the hospital in question and\/or consultant.<\/p>\n\n\n\n<p>If you did not receive a shielding letter, but believe you should have, you are advised to contact your GP.<\/p>\n\n\n\n<p><strong>When will the guidance around shielding be reviewed?<\/strong><\/p>\n\n\n\n<p>Guidance on shielding for people at potentially higher risk from COVID-19 is being <a href=\"https:\/\/www.health-ni.gov.uk\/news\/shielding-will-last-no-longer-necessary-minister\" target=\"_blank\" rel=\"noopener noreferrer\">actively reviewed<\/a>.
